On or about 11/08/2018, at the House located at 15840 SW 43rd St. Century Breeze, Miami FL 33185, the employer did not have a safety program for employees who were exposed to hazards such as but not limited to fall hazard, use of ladders, use of personal protective equipment, and there were no safety inspections of the job site, while they were installing shingles on a roof at a height of approximately 13 feet.
