On or about August 28, 2018, employees were exposed to a struck by hazard while installing roofing materials on a residential apartment building located at 4540 Oak Hill, Amherst, Ohio 44101.  The employees were using air powered nail guns and the employer did not ensure they were wearing protective safety glasses.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about August 28, 2018, employees were exposed to a fall of approximately 23 feet while installing roofing materials on a residential apartment building located at 4540 Oak Hill, Amherst, Ohio 44101.  The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.    Bam Roofing Siding and More was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 994203, citation number 1, item number 1 and was affirmed as a final order on 11/4/2014, with respect to a workplace located at Building C Colonial Gardens, Hurricane, WV 25526.
