29 CFR 1910.22(a)(1) as required by 1960.8(b): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  (a) On or about December 3, 2018 at AAR in Indianapolis, Indiana, the aviation safety inspectors were exposed to tripping hazards from a large number of cords that were located on the floor and in walking areas throughout the hangar.

29 CFR 1910.134(e)(1) as required by 1960.8(b): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    (a) On or about September and October of 2018 at the FAA in Plainfield, Indiana, aviation safety inspectors were provided fit tests on the 3M 6200 respirator, which had been identified as a necessary item for employees who must engage in accident investigations. The employer did not ensure that the employees had been medically evaluated before the fit testing, exposing employees to physical lung damage.

29 CFR 1910.1200(e)(1) as required by 1960.8(b): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) On or about December 3, 2018 at AAR in Indianapolis, Indiana, aviation safety inspectors were potentially exposed to paints, composites, and lubricants in the performance of their inspection duties which potentially exposed them to hazardous chemicals through dermal contact and inhalation.

29 CFR 1910.1020(g)(1) as required by 1960.8(b): Upon an employee's first entering into employment, and at least annually thereafter, each employer shall inform current employees covered by this section of the requirements found in (g)((1)(i) through (g)((1)(iii):    (a) On or about December 3, 2018 at the FAA in Plainfield, Indiana, the employer had failed to annually inform   employees of their right to access medical and exposure records. In addition, they had not been provided right of access to medical and exposure records, such as noise dosimetry, respirator medical evaluations, and audiograms.
