29 CFR 1910.178(a)(4): The employer did not ensure modifications and additions which affect capacity and safe operation of powered industrial truck were not performed by the employer without the manufacturer's prior written approval:  On or about October 1, 2018, employees were exposed to crushing injuries when the employer performed modifications or added non-factory installed attachments to forklifts and the manufacturer did not provide written approval in the following instances:  a.) The Case 588G and Komatsu FG25T-12 forklifts had holes drilled through the forks.  b.) The CAS2531 Cab Lifting Tool was added as an under the forks attachment to the Case 588G forklift.
