29 CFR 1926.503(c): When the employer had reason to believe that any affected employee who had already been trained did not have the understanding and skill required by paragraph (a) of this section, the employer did not retrain each such employee: Instances including, but not limited to, the following: a) Job site 8870 Duncan Avenue, Pittsburgh, PA: On or about November 6, 2018, during carpentry activities, the employer did not ensure that employees used appropriate fall protection systems/devices in that they used retractable devices along an edge in such a manner that they could fall up to approximately 20 feet. b) On or about November 6, 2018, during carpentry activities, the employer provided employees with a self-retracting lanyard ,manufactured by Guardian Fall Protection (MK Edge Series Retractable) for PFAS which was no longer approved for leading edge construction per MFR's Technical Bulletin dated 10/27/17.
