29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:    (a)  Jazz Roofing & Restoration LLC, at 372 Pheasant Run, Louisville, CO: On or about 11/7/18, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  Employees were re-roofing a two story multi-family residential structure.  An employee was not protected from a fall while walking across the roof to a ladder.  The employer did not ensure that all employees were protected by guardrail systems, safety net systems, or personal fall arrest systems.  This condition exposed the employee to an approximate eighteen foot fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program enabling recognition of and appropriate protective procedures for fall hazards for each employee who might be exposed to fall hazards:    (a)  Jazz Roofing & Restoration LLC, at 372 Pheasant Run, Louisville, CO: On and preceding 11/9/18, the employer did not provide a training program enabling recognition of and appropriate protective procedures for fall hazards for each employee who might be exposed to fall hazards.  Employees were re-roofing a two story multi-family residential structure.  The employees had not been provided fall protection training.  This condition exposed the employees to approximate 18 foot fall hazards.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  (a)  Jazz Roofing & Restoration LLC, at 372 Pheasant Run, Louisville, CO: On and preceding 11/9/18, the employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways.  Employees were re-roofing a two story multi-family residential structure.  Employees accessed the roof using extension ladders.  The employer had not provided employees ladder training.  This condition exposed employees to an approximate eighteen foot fall hazard.
