29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) on November 18, 2018, an employee suffered an amputation while working at the wood truss assembly table. The employee's left hand was on top of the truss assembly table near the lift mechanism while reaching under the truss assembly table with his right hand for the switch of the lift mechanism. The lift mechanism immediately amputated the tip of the employee's left index finger when the switch was turned on.
