29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition.  When wet processes are used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided.  On or about August 19,2019 and at times prior thereto, areas of the door plant had wood dust located throughout the floor, built up in front of, behind, and inside equipment cavities, and behind circular handsaw work benches, exposing employees to the hazards of fire, deflagration, and dust explosion.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  On or about August 19,2019 and at times prior thereto, flexible cords (extension cords) were used to supply power from a Norfield Magnum 95, 10 amp accessory outlet, to a Norfield Jamb Router, and a Shop Fox Dust Collector, exposing employees to the hazards of electric shock.
