29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system The employer is failing to ensure that all employees are protected from fall hazards when performing roofing operations. This was most recently observed on November 14, 2018 at 2511 N. 141st Street, Omaha, Nebraska, 68164. Three employees were working on the roof of a residential structure without utilizing fall protection. All three employees were exposed to falls of over 17 feet to the concrete driveway below. The pitch of the roof was greater than 4:12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: The employer is failing to provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. This was most recently observed at the jobsite located at or about 2511 N. 141st Avenue St Omaha, NE 68131 on October 25, 2018. The employer failed to ensure that employees are adequately trained by a competent person in the standards of this subpart resulting in employees failing to utilize proper fall protection.
