29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Gang Mill Machine #1(Machine 65): On or about November 12, 2018 and at times prior, the employer exposed employees to caught in and amputation hazards from unguarded hinge clamps while the operator is instructed to hold metal hinge in place to activate 90 psi pneumatic pressure to hold hinge. b) Gang Mill Machine #2 (Machine 76): On or about November 12, 2018 and at times prior, the employer exposed employees to caught in and amputation hazards from unguarded hinge clamps while the operator is instructed to hold metal hinge in place to activate 90 psi pneumatic pressure to hold hinge. c) Gang Mill Machine #3(Machine 375): On or about November 12, 2018 and at times prior, the employer exposed employees to caught in and amputation hazards from unguarded hinge clamps while the operator is instructed to hold metal hinge in place to activate 90 psi pneumatic pressure to hold hinge.
