29 CFR 1910.253(e)(5)(iv): Hose connections were not clamped or otherwise securely fastened in a manner that would withstand, without leakage, twice the pressure to which they would normally be subjected in service, but in no case less than a pressure of 300 psi:    a)     Coastal Mechanical Systems: On or about 11/14/2018, employees used oxygen and acetylene hoses during cutting and burning operations that were not properly tested after fasteners were installed in that hoses were tested to 90 psi.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI):  a)    Coastal Mechanical Systems: On or about 10/04/2018, an employee welded on stainless steel that according to the safety datasheet contained approximately 18 percent chromium. The employee's 8-hour time-weight average exposure was not determined for Cr6 therefore employee was possibly overexposed to toxic fumes.   Note: The employer is to make this determination in accordance with either paragraph (d)(2), the scheduled monitoring option, or paragraph (d)(3), the performance-oriented option.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)    Coastal Mechanical Systems: On or about 11/14/2018, a DeWalt double insulated 4½-inch angle grinder, serial CMS #AG 514-1, was not used in accordance with the listing and labeling in that the two-prong plug was replaced with a three-prong plug.  No abatement certification or documentation required.
