29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    9 Mohawk Dr., Leominster, MA    As of December 14, 2018 and for years prior, the employer had not performed a periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task.  The employer did not ensure that the inspector reviewed each authorized employee's responsibilities under each energy control procedure.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):      9 Mohawk Dr., Leominster, MA    As of November 9, 2018 and at times prior, the HMT/CSM Tube Bending machine was not sufficiently guarded to prevent employee access to the danger zone during operation.  Although a safety laser scanner was attached, the safety distance was not maximized and included a rear blind spot.  Employees had access to the danger zone when reaching from the blind spot.
