29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Location: Exterior of roofing project @ 53 Howland Rd., Fairhaven, MA  On or about November 1, 2018 employees were exposed to struck-by hazards , where employees were not protected by the use of hardhats from falling debris, tools and construction materials, due to overhead work.

Location: Exterior of roofing project @ 53 Howland Rd., Fairhaven, MA  On or about November 1, 2018 employees were exposed to struck-by hazards, where employees were not protected by the use of safety glasses while using a nail gun.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  Location: Exterior of roofing project @ 53 Howland Rd., Fairhaven, MA  On or about November 1, 2018 employees were exposed to fall hazards of more than 20 feet to the ground below, where employees were not protected by the use of a fall arrest system.  AG1 Construction  Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1070391, citation number 1, item number 1and was affirmed as a final order on 08/26/2015, with respect to a workplace located at 419 Andover Street, North Andover, MA 01845.
