29 CFR 1926.451(g)(4)(iv): When midrails are used, they shall be installed at a height approximately midway between the top edge of the guardrail system and the platform surface. On or about November 15, 2018, employees performing siding installation from a pump-jack scaffold were exposed to a fall hazard of approximately 14.2 feet when the employer failed to ensure that a mid-rail was installed. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On or about November 15, 2018, employees performing siding installation operations on s single-family property were exposed to a fall hazard of approximately 14.2 feet when the employer failed to ensure that the employees were adequately trained to recognize the hazards associated with a fall. Abatement documentation is required of this item in accordance with requirements of 29 CFR 1903.19(d).
