The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about November 5, 2018, the Press Brake Operator of the AccroPress 7608 8 foot 60 Ton Hydraulic Press Brake did not de-energized the Press Brake using lockout/tagout procedures, install the die block or utilize the Press Brake Control System to prevent motion of the Press Brake ram while changing dies.
