29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) On or about October 25, 2018 where the employer failed to report the in-patient hospitalization of an employee who spent three days in Minot Trinity Hospital from October 3, 2018 to October 5, 2018 after a work related accident on a well site four miles East of Bleden Stop off 51st Street NW Bleden, North Dakota.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) On or about October 3, 2018 for the employees exposed to flash fires that were not trained to recognize and mitigate the chemical hazards found in flow back operations at the well site four miles East of Bleden Stop off 51st Street NW Bleden, North Dakota.
