29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:    a) Cattle pen area: On November 14, 2018 and at times prior thereto an inspection of a trench was not conducted by a competent  person to determine the soil type and the proper protective system needed before allowing employees to work in a 6.6 foot deep trench. The trench measured 6.3 to 6.6 feet deep and 3 .5 feet wide.  Note) Certification of corrective action is required for this item.

29 CFR 1928.57(b)(1)(iii): Power take-off driven equipment was not guarded to protect against employee contact with positively driven rotating members of the power drive system:    a) Farm:  On November 14, 2018 and at times prior thereto the shield for the power take off shaft on the #2 feed mixer was broken.  This exposed employees to an 11 inch section of the rotating shaft.            Note) Certification of corrective action is required for this item.

Safety data sheets were not available for chemicals and products which included, but may not be limited to, Klean Strip spray gun paint remover, zero rust, and Buff primer sealer.
