29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    I90 WB Beckett, MA: On or about November 15, 2018 the employer had not initiated and maintained programs which provided for frequent and regular inspections of the job sites, materials and equipment by a competent person such as but not limited to frequent inspections of Fleet vehicles to ensure daily vehicle inspections were examined for accuracy and completeness.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1). 29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    I90 WB Beckett, MA: On or about November 15, 2018 the employer had not instructed each employee in the recognition and avoidance of unsafe conditions and regulations applicable to his\her work environment such as but limited to the control and elimination of back over and struck-by hazards inside roadway work zones.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.601(b)(4)(i): The employer had in use motor vehicle equipment having an obstructed view to the rear which did not have a reverse signal alarm audible above the surrounding noise level:     I90 Westbound, Becket, MA MM17.2: On or about November 15, 2018 the employer had in use a line grinder truck which had an obstructed view to the rear and the reverse signal alarm was not audible above the surrounding noise level.    a) Reverse signal alarm was not audible above surrounding sound.  b) Reverse signal alarm was intermittent.  c) Manual alarm was not connected to reverse signal.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.601(b)(14): All defects were not corrected before motor vehicle(s) were placed in service:      I90 Westbound, Becket, MA MM17.2: On or about November 15, 2018 the employer had in use a line grinder truck which had defects that included but were not limited to non-functioning back up lights, leaking brake air lines, and an internment reverse signal alarm that were not corrected prior to placing such vehicle into service.
