Bridgehampton, NY - An employee was working from a MANITOU Telehandler Truck (rough terrain forklift), Model No. MVT1330LC/Serial No. 157558 on a job-made lift attachment. The MANITOU Telehandler Truck is not designed by the manufacturer to be used as a manlift; on or about 11/15/2018.      Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

Bridgehampton, NY - An employee was working from a MANITOU Telehandler Truck (rough terrain forklift), Model No. MVT1330LC/Serial No. 157558 on a job-made manlift attachment. The employer was using the job-made manlift attachment and did not have the manufacturer's written approval and without the adjusted instruction plates, tags, or decals; on or about 11/15/2018.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

Note: In addition to abatement certification, the employer is required to provide abatement documentation for this item in accordance with 29 CFR 1903.19.
