29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) Gang Rip Saw: On or about November 18, 2018 and at times prior there to, the employer did not ensure hand and arm protection were selected and used while manually handling the blades during a change out which exposed employees to amputations and other injuries associated with making contact with the sharp edges of the blades.
