One of the cables being used to hoist the gable became displaced, fell, and struck an employee.  This condition exposed employees the hazard of being struck-by falling loads.

The signal person was not qualified through the use of a third party evaluator or an employer qualified evaluator. This exposed employees to struck-by hazards.
