29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a. On or about November 13th, 2018, at 636 13th Avenue South Naples FL, 34102, an employee was not knowledgeable in recognizing an approximate 13 feet fall hazard while setting wood from a scaffold and not using a means of personal fall arrest or guardrail system.
