29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) At 1075 Montlimar Drive, Mobile, Alabama (South East Corner - Roof Ledge Utility/HVAC Area): On or about November 15, 2018, the employer allowed his employee to perform HVAC work on a roof ledge without proper fall protection in that a tow strap was used as a lifeline secured through a wall and tied to an A-frame ladder with a knot for an anchor; exposing his employee to a fall hazard of approximately eight (8) feet to the concrete pad or ground below.

29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: a) At 1075 Montlimar Drive, Mobile, Alabama (South East Corner - Roof Ledge Utility/HVAC Area): On or about November 15, 2018, the employer allowed his employee to perform HVAC work on a roof ledge by running his lifeline through a drilled hole from the outside of the building to the inside of the building that was not protected from being damaged or frayed in that the lifeline made direct contact with the siding, sheathing, studs and other materials; exposing his employee to a fall hazard of approximately eight (8) feet to the concrete pad or ground below.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. a) At 1075 Montlimar Drive, Mobile, Alabama (South East Corner - Roof Ledge Utility/HVAC Area): On or about November 15, 2018, the employer allowed his employee to use a tow strap as a lifeline secured through a wall and tied to an A-frame ladder with a knot for an anchor that was not cable of supporting a 5,000 pound (22.2kN) load; exposing his employee to a fall hazard of approximately eight (8) feet to the concrete pad or ground below.
