29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 13, 2018, the employer exposed employees working on the ground or lower edges of the roof to struck-by hazards by not providing hardhats to protect their heads from falling or flying objects such as but not limited to shingles and tools that could cause physical harm.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about November 13, 2018, the employer exposed employees operating nail guns when installing shingles on the roof to struck-by hazards by not providing safety glasses to protect their eyes from flying objects such as but not limited to roofing nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about November 13, 2018, employees wearing climbing belts and installing shingles above 6ft from the ground on a 12/12 pitched rooftop of a single family residential building were exposed to fall from height hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about November 13, 2018, employees did not extend ladders at least 3ft above the eaves of the roof they were installing shingles on.
