29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 16 November 2018, and at times prior thereto, a framer was installing sheeting on the exterior of the building without wearing a hard hat while work was ongoing above, exposing the framer to the hazard of being struck by falling tools, materials, or debris.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 16 November 2018, and at times prior thereto, a framer was using a pneumatic nail gun to apply sheeting to the exterior of a building under construction without the use of eye protection, exposing the worker to the hazard of being struck by flying projectiles or debris.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 16 November 2018, and at times prior thereto, two framers were unloading roof trusses from a forklift while at the edge of the 4-story hotel under construction without fall protection, exposing both workers to the hazard of falling approximately 49 feet to the concrete below.
