29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Employees were exposed to machine caught-between hazards associated with moving parts when they engaged in servicing and maintenance activities on the: a) Line 14 Accumulator b) Line 27 Combi c) Line 38 Bossar's duckbills The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to allowing employees to unjam products on the aforementioned equipment. The machines were not shut down or turned off prior to engaging in this servicing and maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.176(a): Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked: In the Receiving area where forklifts and pedestrians worked, sufficient safe clearances were not allowed for aisles wherever turns and passage must be made. Permanent aisles and passageways were not appropriately marked. A cinderblock structure along the west wall of the Receiving area blocked the path between the marked walkway and a workstation adjacent to an area where forklifts placed pallets of product.
