29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a.  At the work site, employees used a variety of presses and other powered equipment to produce parts for motor vehicles, gardening machinery and lawn care equipment.  On November 9, 2018, an employee was operating the 859 machine that created a point on the end of a metal J-hook.  The employee's left index finger was caught in the clamp during the 859's cycle and amputated.  The employer failed to adequately guard the point of operation.
