29 CFR 1910.28(b)(1)(i):  The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  (i) Guardrail systems; (ii) Safety net systems; (iii) Travel restraint systems; or, (iv) Personal fall arrest systems:    a) Machinex M sorter    Employees who climb on the conveyor to access sorting rollers in MachineX  M sorter were exposed to a fall hazard of approximately 12 feet to the ground below.          Condition observed on or about 11/19/2018
