29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:   a)  At the jobsite, at the rear of the building, on or about 11/15/18:  Employees worked from a ladder jack scaffold where one end of the 20 foot pick extended over one support by over 40 inches, exposing workers to a fall of 12 feet to ground.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  a)  At the jobsite, at the rear of the building, on or about 11/15/18:  Employees worked from a ladder jack scaffold where workers were exposed to a fall of 12 feet to ground at one end of the scaffold without fall protection, where that end of the scaffold extended out over its support by over 40 inches, and where the pick was constructed and used where that working level was not level.  Conditions observed indicated that the scaffold was not erected under the supervision of a competent person.
