29 CFR 1926.652(b)(2): Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart. On or about 11/16/2018, at 5910 Harrison Avenue in Cincinnati, OH, during manhole installation the maximum allowable configurations for benching was not determined for the excavation which exceeded a depth of seven (7) feet.
