29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about 11/19/2018, during an Amputation NEP inspection at AdSell Companies, located at 5001 Southwest Ave., St. Louis, MO, the President and his staff could not produce an established Energy Control Program consisting of energy control procedures, employee training and periodic inspections. Employees serviced and maintained the Komori 6 Color Printing Press (Model; L-640, S/N: 486). This press had electric, pneumatic and water pressure energy sources.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: On or about 11/19/2018, during an Amputation NEP inspection at AdSell Companies, located at 5001 Southwest Ave., St. Louis, MO, the employees were exposed to chemical hazards. The employer did not have a written HazCom Program, or Safety Data Sheets (SDS), nor did the employer provide any type of chemical hazard training. The employees were exposed to one product named RYK 5 Plus Blanket & Roller Wash which exposed employee(s) to an acute health hazard.
