29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof: a) Paint Department: An employee was not removed from the machine area during testing or positioning of machines in that an employee was cleaning the lens of a conveyor camera system when the conveyor was indexed for troubleshooting and the employee's hand was caught by the moving conveyor, on or about November 14, 2018. Abatement Certification Required.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) Paint Department: The onsite employer and an outside servicing contractor (IVC Vision Solutions) did not inform each other of their respective lockout or tagout procedures which resulted in a host employee's hand being caught in a moving conveyor when it was indexed during troubleshooting, on or about November 14, 2018. Abatement Certification Required.
