29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10.    a) On 11/15/2018 an employer did not properly guard the point of operation, two employees were in the process of making picture frame holders utilizing a Power Press (Danly 110 Model # P11001).  Employee #1 was feeding the aluminum strips while Employee #2 was operating the press using two hand controls. While feeding one of the strips into the point of operation, employee #1 reached around the guard to ensure proper feeding, while employee #2 engaged the press.  Employee #1's right hand was crushed as a result of the impact.

29 CFR 1910.217(c)(2)(i)(a): It shall prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard;  a) On 11/15/2018, the guard on the Danly 110 Model #P11001 mechanical power press did not prevent an employee form reaching over, under, around or through the guard to a pinch point area in that when two employees were in the process of operating the press to make picture frame holders, one employee feeding the strips was able to reach around the guard to a pinch point area.

29 CFR 1910.217(f)(2): Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed.    a) On or about 11/20/2018 the employer did not train and instruct press operators in the safe method of work such as but not limited to;  1. How to use all the press controls.  2. Where each safety device is located on the machine and how to use it correctly.   3. How to use tools to remove stuck work and how to use swabs, brushes, or oil cans to lubricate dies and stock.   4. Why, when and how to use personal protective equipment such as safety glasses, gloves, safety shoes and hearing protection.   5. Where to store parts, tools, die sets, bolster plates and other materials so that these items do not present falling hazards.   6. Where possible pinch points with moving components are located.   7. The importance of keeping their work area clean to avoid hazards.  8  Not to operate the press until it has been checked and tested several times prior to production operations and to whom to report problems.
