29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a) On or about November 20, 2018; throughout the site; employees were exposed to a struck by hazard while operating pneumatic nail guns without wearing eye protection.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)    a) On or about November 20, 2018, roof area; employees were working on a residential roof without wearing fall protection exposing them to an estimated 30-35-foot fall hazard.    b) On or about November 20, 2018, roof area; employees wearing fall protection were not tied off, exposing them to an estimated 30-35-foot fall hazard.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a) On or about November 20, 2018; front area; the ladder used to access the roof area was extended eighteen inches above the roof exposing employees to an estimated 30 foot fall.
