On or about November 19, 2018 at Ridgedale Drive and Pinehurst Road, Snellville, Georgia  employees working in an unprotected trench had not been trained to recognize and avoid the unsafe conditions of the trench they were working in.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:(a    On or about November 19, 2018 at Ridgedale Drive and Pinehurst Road, Snellville, Georgia during the excavation to a new sanitary sewer line, the employer failed to provide a safe means of access/egress in the trench approximately 20 feet deep and 30 feet wide.

On or about November19, 2018 at Ridgedale Drive and Pinehurst Road, Snellville, Georgia workers in a trench approximately 20 feet deep were not protected from cave-in hazards when the trench was not sloped or a trench box provided.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.      On or about November 19, 2018 at Ridgedale Drive and Pinehurst Road, Snellville, Georgia where employees were installing a new sanitary sewer line in a trench approximately 20 feet were exposed to a trench collapse when the spoil pile was placed on the edge of the trench.
