29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. a) Lot #50, On or about 11/20/2018, and at times prior, the employer exposed employees to fall hazards of over twenty-five feet, in that the tubular welded frame scaffold with an approximate 5:1 ratio was not secured to the building at any point while employees were conducting bricklaying, and while cleaning the scaffold of tools and materials.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Lot #50, On or about 11/20/2018, and at times prior, the employer exposed employees to hazards of falls of more than twenty-five feet, in that a complete guardrail system was not provided to employees while cleaning the tubular welded frame scaffold of tools and materials.
