On or about November 16, 2018, and times prior thereto, the employer did not ensure lockout/tagout procedures were developed and utilized addressing all potential hazardous energy while performing service/maintenance work activities on the paint conveyor lift. This exposed employees to struck-by and/or caught-in hazards.

29 CFR 1910.147(d)(1): The authorized employee did not have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before the authorized or affected employee turned off the machine or equipment. On or about November 16, 2018, the employer did not ensure the authorized employee performing maintenance work activities on the paint conveyor lift had knowledge of the hazardous energy sources to be controlled. This exposed employees to struck-by and/or caught-in hazards.
