29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      51 Denslow Rd, East Longmeadow, MA: On November 21, 2018 and at times prior, employees performing maintenance activities on the Elsner Machines did not utilize the energy control procedures to isolate and lock all  hazardous energy sources.  On November 21, 2018, an employee locked out one electrical energy source while performing maintenance on an Elsner machine and the procedure identified three electrical and one pneumatic sources of energy for the machine.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    51 Denslow Rd, East Longmeadow, MA: As of November 21, 2018, the employer had not performed an annual periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task.  The employer did not ensure that the inspector reviewed each authorized employee's responsibilities under each energy control procedure.
