29 CFR 1910.219(c)(4)(i): Unguarded shaft end(s) projected more than one half the diameter of the shaft:      (a) On or about November 15, 2018, and at times prior, the employer did not ensure that employees were protected from caught-in hazards while working next to unguarded end shafts that projected more than one half the diameter of the shaft. Employees were exposed to caught-in hazards in that at least 5 unguarded projecting shafts were observed in the Nigrelli Conveyor system located in the upper hall packaging area. 29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  (a) On or about November 15, 2018, and at times prior, the employer did not ensure that each exit was marked by a sign reading "Exit". Employees in the Annex room were exposed to fire-related hazards in that the two emergency doors in the room were not marked by Exit signs. Employees were exposed to contact with electrical hazards in that the employer used a flexible cord as a substitute for permanent wiring on a 220V blower motor mounted on a wall behind vessel TDV7. 29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v):  (a) On or about November 15, 2018, and at times prior, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by the hazard communication standard. Employees in the brew deck area were exposed to contact with chemical hazards in that a 5-gal container with a Sodium Hydroxide solution was not labeled. The Sodium Hydroxide solution is used for cleaning purposes and is a known corrosive agent.
