29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about November 16, 2018, at a job-site located at 2472 Wise Ave NW, Canton, Ohio 44708; the employer did not ensure an employee of a subcontractor were protected from a cave in hazard by utilizing adequate cave-in protection to protect an employee working inside a trench with near vertical side walls with a depth of approximately 6.7 feet.
