29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219(f)(3): JOB SITE: 9 Industrial Drive, Saint James, MO 65559, on or about 11/20/2018, workers were exposed to an unguarded sprocket and chain hazard while operating the Evans Rotork, Laminate Press machine, Model: 0206, S/N: 696810. Workers were exposed to pinch point hazard while operating and feeding the material into the press.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: JOB SITE: 9 Industrial Drive, Saint James, MO 65559, on or about 11/20/2018, a check of the 110v electrical power box serving the Evans Rotork Laminate Press revealed unused openings were not effectively closed. The box was missing two (2) knockout pins.
