29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.    On or about November 27, 2018, employees were potentially exposed to eye injuries while operating band saws, CNC, and manual lathes. The employer did not require the use of prescription safety glasses and/or eye protection that could be worn over the prescription glasses as required by company's policy and hazard assessment.     There is no abatement certification or documentation required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    On or about November 27, 2018, an employee was exposed to struck-by and caught-in hazards, such as flying chips and clothing caught in rotating parts, while operating a South Bend manual lathe. The employer did not ensure guarding was installed to protect the operator from the rotating parts and flying debris created during operation.    There is no abatement certification or documentation required for this item.
