29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) 4000 Forge Hammer: The employer did not insure that the Pneumatic Key Driver used for die installation, was  properly guarded to protect employees from the hazards associated with a pneumatic hammer. This exposed employee(s) working on or around the Pneumatic Key Driver to struck by hazard, pinch points, flying projectiles and amputation injury.

29 CFR 1910.218(a)(2)(i): Establishing periodic and regular maintenance safety checks and keeping certification records of these inspections which include the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, for the forging machine which was inspected.    (a) 4000 Forge Hammer: The employer did not insure that periodic and regularly scheduled maintenance safety checks had been performed on forge shop equipment, such as the 4000 Forge Hammer and Pneumatic Key Driver used for Die installation on the forge hammer. This exposed employee(s) working on or around the Pneumatic Key Driver and forge machines(s) to struck by hazard, flying projectiles, pinch point hazard and amputation injury.

29 CFR 1910.218(b)(1): Die keys and shims were not made from a grade of material that will not unduly crack or splinter.    (a) 4000 Forge Hammer: On or about December 11, 2018, die keys used in the process to secure forge dies by insertion were made from 4340 alloy steel and struck by a 4340 steel alloy arbor attached to the pneumatic key driver as part of the die setting process. This caused the keys and shims to be exposed to forces that can cause cracking and splintering, exposing employees working on and around the Forging Die and associated die keys to struck by hazard from flying projectiles.
