29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103) At the QT Project: On or about November 27, 2018 and at times prior thereto, the employees exposed to cement dust when mixing Spec Mix were given an N95 filtering face piece respirator without having a medical evaluation.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: At QT Store Project: On or about November 27, 2018, and at times prior there to, employees were exposed to chemicals such as but not limited to Spec Mix which contains silica, while mixing cement without being provided safety glasses and wear imperative gloves such as nitrile.
