On or about November 21, 2018 on the grain leg of the upper silo dry bean leg elevator a new platform was being constructed an employee of a subcontractor was installing guard rails when his retractable lanyard caught in the rotating shaft of the grain leg.  The shaft was not guarded and located at floor level on the platform.
