Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Pizza Hut, 941 W. South Blvd., Montgomery, AL: On November 28, 2018 and at times prior to, the employer exposed employees to fall hazards of up to 8 feet by failing to ensure employees used a fall protection system or guardrail system on a 2/12 pitch roof while conducting roofing work tasks.
