29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about November 28, 2018:   a. On the north side of a commercial building with a steep roof where two employees were exposed to a fall hazard exceeding 6 feet when performing roofing work without a fall protection system.   b. On the east side of a commercial building with a steep roof where one employee was exposed to a fall hazard exceeding 6 feet when performing roofing work without a fall protection system.

On the north side of a commercial building where two employees were exposed to a fall hazards when performing roofing work without a training program to enable each employee to recognize the hazards and procedures to be followed in order to minimize the hazards.   b. On the east side of a commercial building where one employee was exposed to a fall hazards when performing roofing work without a training program to enable each employee to recognize the hazards and procedures to be followed in order to minimize the hazards.
