29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports.  Location: 331 Pine St, Jersey City, NJ.  a) An employee involved in form stripping operations at the site's garage entrance was working from a tubular welded scaffold that was not fully planked at the working level.  The employee was exposed to a potential fall hazard of approximately 6 ft. to the lower level; on or about 11/19/18.   Concrete Systems, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1055944, citation number 1, item number 1 and was affirmed as a final order on 2/8/2016, with respect to a workplace located at 535 Mountain Ave, New Providence, NJ.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  Location: Location: 331 Pine St, Jersey City, NJ.  a) Employees were observed using the cross braces of a tubular welded scaffold as means of access and/or egress during the elevator shaft construction; on or about 11/19/17.  b) An employee was observed using the cross braces of a tubular welded scaffold as means of access and/or egress at the site's lobby; on or about 11/19/17.  Concrete Systems, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 963987, citation number 1, item number 1A and was affirmed as a final order on 11/6/2014, with respect to a workplace located at 300 Glenwood Ave., Bloomfield, NJ.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee were not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Location: 331 Pine St., Jersey City, NJ.    a) An employee was observed working from the top level of a tubular welded scaffold during elevator shaft construction but the scaffold was not provided with a guardrail system and the employee did not have a personal fall arrest system.  The worker was exposed to a potential fall hazard of approximately 20 ft to the concrete floor below; on or about 11/19/18.    Concrete Systems, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vii), which was contained in OSHA inspection number 963987, citation number 1, item number 2A and was affirmed as a final order on 11/6/2014, with respect to a workplace located at 300 Glenwood Ave., Bloomfield, NJ.

Concrete Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(1), which was contained in OSHA inspection number 1190281, citation number 2, item number 1, and was affirmed as a final order on 8/6/2018, with respect to a workplace located at 354-364 New York Ave., 54-62 Houston St. & 48-57 Napoleon St., Newark, NJ.
