29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    TGM, LLC - Bryan, Ohio:  On or about November 29, 2018, the employer did not ensure that procedures for the control of hazardous energy were developed, documented, and utilized for servicing and maintenance activities on mills and lathes, (including the Okuma "A" Mill - E-Series, ES-V4020; S/N 1241), which can expose employees to hazards associated with rotating and moving parts while performing maintenance and servicing.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    TGM, LLC - Bryan, Ohio:  On or about November 29, 2018, the employer did not ensure that periodic inspections were performed to ensure that lockout-tagout procedures were being followed for servicing and maintenance activities on equipment, such as but not limited to the Okuma "A" Mill - E-Series (ES-V4020; SN 1241).  Lack of following lockout-tagout procedures can lead to exposure to hazards involving rotating and moving parts.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  TGM, LLC - Bryan, Ohio:  On or about November 29, 2018, the employer did not ensure that employees who were assigned to perform maintenance and servicing work on mills and lathes were trained in the application, usage and removal of lockout-tagout equipment.  Employees were exposed to the potential of moving and rotating parts while performing servicing and maintenance activities on equipment, such as but not limited to the Okuma "A" Mill - E Series (ES-V4020; S/N 1241).
