29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about November 29, 2018, near 12807 FM 1960 W., where employees were exposed to a struck-by hazard when working in an excavation without all materials being kept at least two feet from the edge of the excavation.
